Some people who have Medicare also have Medicaid and are entitled to enroll in a Dual Eligible Special Needs Plan (also called D-SNP).  These plans add to the member's existing benefits with no extra cost to the member.  Each D-SNP plan contracts with Medicare & the State Medicaid program in which they offer the D-SNP plan.  The extra benefits can include Over the Counter allowances, Health Food Grocery benefits, Utility Assistance, Transportation, Personal Emergency Response systems, Fitness memberships, Home Delivered meals, In home supports, In home chore services, Assistance devices, Home Modifications, Concierge services, Service Animal support, Robotic Companion Pets,  Pest Control, Acupuncture and additional Dental, Vision & Hearing coverage.  Plans can also provide Case workers & Care Management for at risk members.  The plans provide all of the Medicare Part A, B, & D services and in some states also provide the Medicaid benefits.  Plan designs can be either PPO or HMO depending on the county & state.
